Home nursing jobs

Some of Abcor’s home care clients require more medical attention than a typical caregiver can legally provide. To assist these clients, Abcor also employs a variety of home health professionals. Job Description

 The Homecare Supervisor is responsible for the following:

  • Accepting Cases from the Illinois Department on Aging (CCU) and Illinois Department of Human Services, affiliated programs with ABCOR and Private Sources.
  • Monitoring provision of services according to authorized client’s Plan of Care
  • Monitoring active and on hold clients for status changes
  • Preparing service calendars
  • Answering phones assisting clients with questions
  • Inactivating Field Staff/Training Field Staff
  • Monitor field staff compliance with mandatory in-service training sessions
  • Providing input to annual homemaker’s / CNA performance evaluations
