Care Coordinator – Full Time
Accepting Cases from the Illinois Department on Aging (CCU) and Illinois Department of Human Services, affiliated programs with ABCOR and Private Sources.
2. Monitoring provision of services according to authorized client’s Plan of Care
3. Monitoring active and on hold clients for status changes
4. Preparing service calendars
5. Answering phones assisting clients with questions
6. Hiring Field Staff / Inactivating Field Staff
7. Training Field Staff
8. Monitor field staff compliance with mandatory in-service training sessions
9. Providing input to annual homemaker’s / CNA performance evaluations
10. Providing Homemakers / CNA with schedule, monitoring receipt procedures in the conduct of essential shopping and errands as stated in the Plan of Care, monitoring required documentation (timesheets) to support units of service requested for reimbursement shall be retained in paper and submitted to billing department
11. Provide emotional support and encouragement to clients and homemakers
12. Assisting with in-service training
13. Weekly face-to-face or over the phone homemaker conferences
14. Supervise field staff compliance with timesheets and attendance
15. Processing field staff timesheets for billing / Payroll purposes
16. Making client’s in-home visits, as necessary, to provide hands-on training and assistance in the client’s care
17. Acting as liaison between client homemaker / CNA and the CCU
18. Developing appropriate skills and attitudes on the part of the homemaker to enable the homemaker to best serve customers.
19. Maintain client personal files by documenting client contact and activities related to client services in the client’s file.
20. Give reports daily activity to the Homecare Manager
21. Participate in daily office staff conferences discussing and learning important ponts in case management.
22. Assist Homecare Manager and Administrator in the office projects.
23. Update client’s and field staff schedules
24. Inform homecare aid of the new client assessments over the phone or in person
25. Conference and coordination of care with other agencies and facilities present in the clients care
26. Initiating and / or participating in client staffing discussions with the CCU’s case manager
27. Providing input to the CCU’s case manager on the services that are needed for each client as a result of conferences with the homecare aid or in-home visits.
28. Resolve family-client conflicts
29. Prepare written report for each active case monthly
30. Maintain POC Compliance % of > 85%
31. Quarterly review participation by preparing reviewing reports, planning preparing and documenting contact and quarterly face-to-face conferences with each assigned homecare aid
