Check your eligibility for free Home Care ServicesWe are contracted with several payers including the Illinois Department on Aging, the Illinois Department of Human Services, and managed care organizations such as Aetna, Blue Cross Blue Shield, County Care (Cook county only), Humana, Meridian, and Molina. Older Adults who meet the established eligibility criteria are approved for subsidized in-home services.

Who is eligible?

60 years old or older

Either U.S. citizens or eligible non-citizens within the specific categories

Residents of Illinois

Have non-exempt assets of $17,500 or less (Your home, car, or personal furnishings are classified as exempt assets.

Have an assessed need for long-term care (to be at risk for nursing facility placement as measured by the Determination of Need (DON) assessment).
